I understand that participation in AWANA carries certain physical risks, among others: accidents which cannot be foreseen and personal injury, including but not limited to death or sickness from contracting the Coronavirus disease 2019 (“COVID‐19”), the infectious disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). As a parent/guardian, I do herewith authorize participation in activities and medical treatment when required under the direction of any licensed physician of the above minor in the event of a medical emergency, which, in the option of the attending physician, may endanger his or her life, cause disfigurement, physical impairment, or undue discomfort if delayed. This authority is granted only after a reasonable effort has been made to reach me by phone at the numbers listed above or my designated emergency contact below. I understand that club activities such as picnics, swim parties, skating, etc. may not necessarily take place on church premises. I give permission for my child(ren) to participate in these activities of my own free will. This includes riding/carpooling to and from Club and to different activities.
The undersigned assumes the responsibility for any legal liability or costs connected with such medical treatment as may be required and hereby release Awana Clubs International, the church where the child attends AWANA club and their representatives from any liability thereof.

Our media team will be taking photos and videos of the AWANA clubs throughout the year. By paying for this you are granting permission for your child's photo/video to be published on Rock-Land Baptist Church website, Facebook/Instagram page, YouTube channel, and used in other events and published materials.
